Summary Statement of Deficiencies D6120 TECHNICAL SUPERVISOR RESPONSIBILITIES CFR(s): 493.1451(b)(7)(8) (b)(7) Identifying training needs and assuring that each individual performing tests receives regular in-service training and education appropriate for the type and complexity of the laboratory services performed; (b)(8) Evaluating the competency of all testing personnel and assuring that the staff maintain their competency to perform test procedures and report test results promptly, accurately and proficiently. This STANDARD is not met as evidenced by: Based on a review of testing records, competency assessments, and an interview with the technical supervisor, the laboratory failed to provide training and assess testing personnel competency prior to reporting patient results for urine sediment examinations for two (2) of two (2) testing persons. Findings include: 1. Urine sediment records reveal the laboratory began reporting urine sediment examinations on 10/14/2024. 2. A request was made to review urine sediment examination training records and documentation could not be provided. 3. A review of the competency assessments records for two of two testing persons revealed the assessments were performed on 1/7/2025. 4. An phone interview with the technical supervisor conducted on 1/13/2025 at 4:00 PM confirmed these findings. 5. The laboratory reports performing 560 urine sediment examinations during the period between 10/14 /24 and 1/7/2025.
