This STANDARD is not met as evidenced by: Based on a lack of personnel license, a review of the Tennessee Medical Laboratory Board licensure verification web page, and interview with the lead testing person, the laboratory performing high complexity testing employs one person who is not licensed as required by the State of Tennessee. Findings include: 1. There were no licenses obtained for one testing personnel. 2. The Tennessee Medical Laboratory Board web page confirmed the lack of license for one testing person as required by the State of Tennessee for employees working in a State of Tennessee licensed laboratory.. 3. An interview with the lead testing person confirms at 10:00 AM on September 20, 2018 there was no license obtained for one testing person.
