This STANDARD is not met as evidenced by: Based on record review and interview, the laboratory's Technical Consultant failed to perform competency evaluations on testing personnel who performed potassium hydroxide (KOH) and Scabies testing for 2 out of 4 moderate level testing personnel (A and B) in 2018 Findings: Review of the "Laboratory Personnel Report" signed by the laboratory director on 3/12/19 showed that there are four moderate level testing personnel. Review of employee competencies revealed that there was no competency evaluations on moderate Testing Personnel A and B for 2018 During an interview on 1 /12/19 at 9:09 AM, the Clinical Laboratory Administrator acknowledged that a competency evaluation was not performed in 2018 on moderate Testing Personnel A and B.
