Summary Statement of Deficiencies D6054 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least annually, after the first year. This STANDARD is not met as evidenced by: . Based on lack of documentation and interview with the Office Manager (OM), Technical Consultant #2 (TC2) failed to evaluate the competency of Testing Personnel #1 (TP1) for 2 (October 2017 to October 2019) of 2 years. Findings include: 1. Competency evaluations revealed lack of documentation for the competency of TP1 performing the moderately complex synovial fluid presence /absence of crystals for 2 of 2 years. 2. On October 3, 2019 at approximately 10:00 am when queried, the OM was unaware that TP1 needed to be evaluated. 3. During the interview on October 3, 2019 at approximately 10:00 am, the OM acknowledged competency evaluations had not been performed and documented on TP1 for 2 of 2 years of testing.
