Summary Statement of Deficiencies D6054 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least annually, after the first year. This STANDARD is not met as evidenced by: Based on record review and interview with laboratory personnel, the technical consultant did not document competency of the one testing person for one year. Findings include: Review of personnel records on 01/17/19 revealed that there were annual competency evaluations for the testing person for 2016, 2017, and in 2019, but not for 2018. During an interview with the testing person at 11:00 a.m. on 01/17/19, he said that he thought they needed to be done every other year.
