Summary Statement of Deficiencies D0000 A recertification survey conducted on 09/09/2020 found that Barnett Dermatology PA clinical laboratory was not in compliance with 42 CFR Part 493, Requirements for Laboratories. D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based on record review and staff interview, the laboratory failed to have initial competency assessment for 1 (TP A) out of 3 testing personnel (TP) (A, B, C). Also, there was no semi-annual competency assesment for 2 (TP A, TP B) out of 3 (TP A, TP B and TP C) from 1/2019 to 9/2020 . The findings include: -A review of personnel competency assessment records for 2019 and 2020 revealed that for TP A, there was no documentation of initial and semi-annual competency since May 2019; for TP C there was no 6 month competency assessment documented between 8/27/2018 and 1/3 /2020. During an interview on 09/09/2020 at 10:00 AM, the laboratory consultant confirmed that the laboratory failed to have documentation of the above listed competency assessments.
