Summary Statement of Deficiencies D0000 A recertification survey conducted on 5/16/22 found that Beal Dermatology Pllc, clinical laboratory located in Jacksonville, Florida, was NOT in compliance with 42 CFR Part 493, Requirements for Laboratories. D5217 EVALUATION OF PROFICIENCY TESTING PERFORMANCE CFR(s): 493.1236(c)(1) At least twice annually, the laboratory must verify the accuracy of any test or procedure it performs that is not included in subpart I of this part. This STANDARD is not met as evidenced by: Based on record review and staff interview, the laboratory failed to send histopathology patient slides for peer review more than once annually in 2021. Findings include: The record review of documented peer review for 2021 showed that patient slides were sent on 3/10/2021 to a Dermatopathologist to verify accuracy of results. There was no other documented peer review for 2021. The interview with the Laboratory Director on 5/16/22 at 9:30am confirmed that slides were only sent out once for peer review in 2021. .
