This STANDARD is not met as evidenced by: Based on a review of the Mohs technicians yearly competency evaluations and an interview with the office manager, three out of three specimen processors competency evaluations did not contain the six elements of competency as mandated by the Centers for Medicare and Medicaid Services (CMS). See attached. Findings include: 1.
