Summary Statement of Deficiencies D0000 An announced CLIA recertification survey was conducted at The Center for Skin Cancer Surgery on April 22, 2026. The laboratory was surveyed under 42 CFR Part 493 CLIA requirements. Standard deficiencies cited are as follows: D3011 FACILITIES CFR(s): 493.1101(d) Safety procedures must be established, accessible, and observed to ensure protection from physical, chemical, biochemical, and electrical hazards, and biohazardous materials. This STANDARD is not met as evidenced by: Based on interview, review of the laboratory's procedure manual, and review of the safety data sheets (SDS), the laboratory failed to ensure protection from chemical hazards used in their Hematoxylin and Eosin (H&E) stain from 03/01/2024 to 04/22 /2026. Findings Included: 1. Review of the procedure titled, Chemical Preparation for Tissue Processing with Hematoxylin and Eosin noted the laboratory used the following chemicals in their H&E stain: 10% Formalin, Hematoxylin, Clarifier, Bluing Reagent, Eosin, and 100% Alcohol. 2. Review of the SDS for the Epredia Hematoxylin 7211, Fisher Scientific Clarifier 2, Epredia Eosin - Y Alcoholic, and Mercedes Scientific 100% Reagent Alcohol noted, "Dispose of contents/container to approved waste disposal plant." 3. During an interview on 4/22/2026 at 9:43 AM, the Mohs Technician stated they dumped the chemical from the H&E stain down the drain.
