Summary Statement of Deficiencies D6046 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(8) (b) The technical consultant is responsible for-- (b)(8) Evaluating the competency of all testing personnel and assuring that the staff maintain their competency to perform test procedures and report test results promptly, accurately and proficiently. This STANDARD is not met as evidenced by: Based on Competency Assessment (CA) record review and interview with the Laboratory Director, Technical Consultant (TC) #2 and Technical Consultant #3, Technical Consultant #1 (Laboratory Director) failed to evaluate the competency of all testing personnel as required from 2016 to 2017. Findings include: 1. The Provider Performed Microscopy Competency Assessments records reviewed on the day of inspection (01/10/2018) revealed that 6 of 6 testing personnel (TP) who perform KOH, Wet Mount and Fern testing were assessed by individuals not listed on the CMS 209 form as a Technical consultant (TC#1). 2. TC #2 and 3 confirmed the findings above on 01/10/2018 around 12:00 PM.
