Summary Statement of Deficiencies D2007 TESTING OF PROFICIENCY TESTING SAMPLES CFR(s): 493.801(b)(1) The samples must be examined or tested with the laboratory's regular patient workload by personnel who routinely perform the testing in the laboratory, using the laboratory's routine methods This STANDARD is not met as evidenced by: Based on a review of the College of American Proficiency(PT) records and an interview with the lead laboratory testing person, the laboratory failed to rotate hematology proficiency test specimens among all four testing personnel in 2017 and the first event in 2018. Findings: The lead laboratory testing person confirmed with this surveyor on April 26, 2018, at approximately 1:30 PM that the laboratory has four staff members who routinely perform patient hematology testing in the laboratory. All CAP Hematology PT events in 2017 and the first event in 2018 was performed by the lead testing person.
