Summary Statement of Deficiencies D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based on a record review and an interview with the laboratory's quality manager (QM), the laboratory failed to establish and follow a policy to evaluate technical consultant competency, for two of two technical consultants (TC), since the last survey on 03/11/2021. Findings include: 1. A review of the laboratory's competency records revealed the laboratory failed to write and follow a policy to assess the competency of its TCs. 2. An interview with the QM on 01.23.24 at 1:20 p.m., confirmed that the laboratory did not have a written policy for competency assessments for the TCs and did not evaluate TC competency.
