Summary Statement of Deficiencies D6054 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least annually, after the first year. This STANDARD is not met as evidenced by: Based on surveyor review of personnel competency evaluations and interview with the current technical consultant, staff A, the previous technical consultant did not document the performance of competency for two of two testing personnel annually for 2021 and 2022. Findings include: 1. Review of personnel competency evaluations showed the following: Staff B: Initial and semiannual competency in 2020. Further review showed no additional competency evaluations were documented in 2021 or 2022. Staff C: Initial competency in 2020 and semiannual competency in January 2021. Further review showed no additional competency evaluations in 2021 and 2022. 2. Interview with staff A on March 6, 2023, at 1:25 PM confirmed the previous technical consultant did not document competency performance for two of two testing personnel for the last two years.
