This STANDARD is not met as evidenced by: . Based on record review and interview with the Testing Person 1 (TP1) on 5/24/21, the Technical Consultant (TC) failed to evaluate and document the performance of individuals responsible for moderate complexity testing at least annually, after the first year as evidenced by the following: The surveyor reviwed the personnel competency records for 2020 and 2021on 5/24/21. The review revealed that an annual competency evaluation was not performed and documented at least anually for two (2) testing personnel after the first year. TP1 confirmed in an interview on 5/24/21 at 11: 58 A.M. that the TC failed to perform and document annual competency evaluations for TP1 and TP6 after the first year.
