Summary Statement of Deficiencies D1002 REPORTING OF SARS-CoV-2 TEST RESULTS During the Public Health Emergency, as defined in 400.200 of this chapter, each laboratory that performs a test that is intended to detect SARS-CoV-2 or to diagnose a possible case of COVID-19 (hereinafter referred to as a "SARS-CoV-2 test") must report SARS-CoV-2 test results to the Secretary in such form and manner, and at such timing and frequency, as the Secretary may prescribe. This CONDITION is not met as evidenced by: Based on interview with the Executive Director and review of COVID testing records, the facility failed to report all COVID test results to the Oregon Health Authority (OHA). Findings include: 1. During interview with the Executive Director of Countryside Living Canby on 08/26/2021 at approximately 0930, she verified that neither positive or negative results from the Binax Now Antigen test had been reported to the OHA since the beginning of testing in October 2020 until the date of survey 8/26/2021. 2. During the time period from October 5, 2020 and August 26, 2021, eight hundred and twenty three (823) COVID tests were performed using the Abbott NOW antigen test. Of these 823 tests, seven hundred ninety eight (798) were negative and twenty five (25) were positive.
