Summary Statement of Deficiencies D2127 HEMATOLOGY CFR(s): 493.851(d) Failure to return proficiency testing results to the proficiency testing program within the time frame specified by the program is unsatisfactory performance and results in a score of 0 for the testing event. This STANDARD is not met as evidenced by: Based on review of 2017, 2018, and 2019 CAP (College of American Pathologists) proficiency testing records and interview with the TC (technical consultant) 3/28/19, the laboratory failed to submit proficiency testing results within the timeframe specified by CAP for the 2019 FH2-A test event, resulting in a score of 0%. Review of 2017, 2018, and 2019 CAP proficiency testing records revealed the submission deadline for the 2019 FH2-A hematology test event was 2/19/19, no later than midnight central time. Review of records revealed the laboratory tested the proficiency samples 2/9/19, but the results were not submitted until 2/22/19. A notation in the records indicated that the laboratory attempted to submit results 2/18 /19 but had problems with submission. The proficiency testing company received the laboratory's results after the due date, and the laboratory received a score of 0% for the test event. During interview at approximately 2:20 p.m., the TC confirmed that the laboratory's results were submitted after the deadline. This deficiency was cited on the previous survey 2/22/17.
