Summary Statement of Deficiencies D6053 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least semiannually during the first year the individual tests patient specimens. This STANDARD is not met as evidenced by: Based on record review and interview, the technical consultant failed to evaluate and document at least semiannually the performance of Testing Personnel A during the first year of employment. Findings: The Laboratory Director serves as the technical consultant for the laboratory. Review of training and competency forms for Testing Personnel A showed that the training form and competency form were signed by the office manager on 1/16/18. No other evaluations of Testing Personnel A were available for review. The laboratory director's signature was not on the training or competency forms. During an interview on 12/07/18 at 9:40 AM, Testing Personnel A stated she started working in the laboratory in the fall of 2017. During an interview on 12/07/18 at 9:45 AM, Office Manager acknowledged that they did not know that competency needed to be completed at least semiannually during the first year of employment and that the laboratory director needed to perform the evaluation and sign the forms.
