Summary Statement of Deficiencies D0000 An offsite COVID survey was conducted for Dennis G. Walker Family Clinic, CLIA #19D2232048 on August 9, 2022. In interview on August 9, 2022 at 02:24 pm, the Office Manager with Dennis G. Walker Family Clinic stated the facility started patient testing around October of 2021 and that a policy was in place for patient testing and reporting patient test results to state agency. She further stated the facility utilizes the IHealth Covid-19 Antigen Rapid test kits for patient testing. The Office Manager emailed the requested documents for confirmation that the facility is reporting COVID 19 patient test results to the State of Louisiana Department of Health as required by the CMS. The laboratory was found in compliance with 42 CFR 493 Requirements for Laboratories. No deficiencies were cited.
