Summary Statement of Deficiencies D6094 LABORATORY DIRECTOR RESPONSIBILITIES CFR(s): 493.1445(e)(5) The laboratory director must ensure that the quality assessment programs are established and maintained to assure the quality of laboratory services provided and to identify failures in quality as they occur. This STANDARD is not met as evidenced by: Based on record review and interview with the histotechnician, the laboratory director did not ensure that the quality assurance review for the laboratory maintenance logs was performed, signed and dated by the laboratory director. Findings: 1. The Laboratory Maintenance logs for both all twelve months of both 2020 and 2021 were not signed and dated at the bottom showing review by the laboratory director; and 2. This was confirmed during interview with the histotechnician during interview at 11: 00 am, the day of survey.
