This STANDARD is not met as evidenced by: Based on observation of slide storage and interview with laboratory staff, the laboratory failed to store slides under conditions that ensure proper preservation. Findings: 1. Observation of the slide storage area revealed a car garage with no temperature-controlled environment. Slides were exposed to extreme hot and cold environments. 2. Interview with laboratory staff on September 14, 2018 at 9:30 AM confirmed the laboratory failed to store histopathology slides under conditions that ensure proper preservation.
