This STANDARD is not met as evidenced by: Based on review of the laboratory's list of tests performed, lack of verification records, and interview with testing personnel #14, as indicated on the CMS-209, the laboratory failed to have a system for verifying the accuracy of histology biopsy testing at least twice annually for 2023 and 2024. Findings are: 1. Review of the laboratory's list of tests performed revealed the laboratory performs histology biopsy testing. 2. Interview with testing personnel #14, as indicated on the CMS-209, on 4/23 /2025 at 11:44 AM confirmed the laboratory did not perform accuracy verification for histology biopsy testing in 2023 and 2024.
