Summary Statement of Deficiencies D6054 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least annually, after the first year. This STANDARD is not met as evidenced by: Based on review of the annual competency records and interview with the testing person, the technical consultant/laboratory director did not ensure that all testing personnel received an annual competency review. Findings: 1. The laboratory currently has 2 testing personnel listed on the "Laboratory Personnel Report (CLIA)" form. 2. The annual competencies for 2018 and 2017 were reviewed. The records showed that one of the two testing personnel did not have an annual evaluation for 2018 and 2017. 3. During the survey on 06/18/18 at 12:45 PM the testing person confirmed that the annual competency had not been performed for two years for the other testing person at the laboratory.
