This STANDARD is not met as evidenced by: Based on surveyor review of the Personnel Files (PF) and interview with the Laboratory Director (LD), the LD failed to have foreign education equivalency for one out of one testing personnel from 8/8/16 to the date of survey. The LD confirmed on 8/15/18 at 1:50 pm that foreign education evaluation was not in the PF.
