This STANDARD is not met as evidenced by: Based on observation and interview with the clinic supervisor, the laboratory failed to ensure expired supplies were not available for use. Findings include: (1) Observation of the laboratory on 10/23/2025 at 09:30 am, identified the following expired supplies were available for use: (a) Fifty BBL Culture Swab Plus collection and transport system, Lot 231507100, Expired on 08/18/2025; (b) One bottle of Avantik CoverMount, Lot 173125, Expired on 04/08/2025; (c) One bottle of Avantik Tissue Marking Dye (Blue), Lot 180073, Expired on 09/30/2025. (2) Interview with the clinic supervisor on 10/23/2025 at 09:30 am confirmed the expired supplies were available for use.
