This STANDARD is not met as evidenced by: Based on record review and laboratory staff interview, the facility failed to submit proficiency testing results before the due date for one of six American Proficiency Institute (API) testing events reviewed. The findings include: The record review on 2/7 /18 of the API 2016 Chemistry Group 2 - 1st event showed a score of 0% for the following analytes: HCG Serum, Albumin, Alkaline Phosphatase, Alanine Aminotransferase, Amylase, Direct Bilirubin, Gamma-glutamyl transferase, Potassium, Urea Nitrogen, Total Bilirubin, Total Calcium, Chloride, Glucose, Sodium, Uric Acid, Carbon Dioxide, Creatine Kinase, Creatinine, Magnesium, and Total Protein. The interview with the laboratory manager on 2/7/18 at 12:00pm confirmed the failure to participate due to not submitting results before the time frame had expired.
