This STANDARD is not met as evidenced by: Based on record review and interview with Testing Person # A (TPA), the laboratory failed to verify the accuracy of frozen sections slides twice annually for 2 out of 2 years. Findings include: Review of verification accuracy records on 9/4/2018 showed that for year 2017 only one performed and for year 2018 from January to August no accuracy evaluation done During and interview with the TPA at 11:00 a.m. on 09/04 /2018 confirmed that there was no documentation to indicate that verification accuracy done twice during 2017 and for 2018 no accuracy verification performed from January to August.
