This STANDARD is not met as evidenced by: Based on record review and staff interview, the facility failed to receive a passing proficiency test score for the third testing event of 2016 for the specialty of Hematology. Findings: Review of the American Proficiency Institute testing results for the third event of 2016 showed a score of 0% for White Blood Cell Differential, Granulocytes, Lymphocytes, and Monocytes. During an interview on 7/25/18 at 12:30 PM, the Laboratory Supervisor confirmed the laboratory had failed the proficiency testing due to clerical error.
