Summary Statement of Deficiencies D0000 An announced CLIA recertification survey was conducted at Florida Center for Dermatology Laboratory II on 7/9/2026. The laboratory is not in compliance with 42 CFR Part 493, Requirements for Laboratories. The following is a description of the standard level deficiencies: D3007 FACILITIES CFR(s): 493.1101(b) (b) The laboratory must have appropriate and sufficient equipment, instruments, reagents, materials, and supplies for the type and volume of testing it performs. This STANDARD is not met as evidenced by: Based on observation and interview, the laboratory failed to ensure that it maintained appropriate and sufficient equipment necessary for accurate and reliable test system operation by failing to have a thermometer available in the Mohs laboratory to monitor required environmental conditions. Findings include: Observation on 07/09 /2026 at 9:15am revealed that no thermometer was present in the Mohs laboratory. Although the laboratory's policy and the "Room Temp/Humidity Log" required the daily recording of room temperature and humidity, the equipment necessary to obtain these measurements was not available on the premises. In an interview on 07/09/2026 at 10:00 AM, Staff #A (Mohs Technician) confirmed that there was no thermometer currently in the laboratory. Staff #A stated that the thermometer is "usually here" and further noted that "it gets warm in the laboratory usually around 2 PM."
