Summary Statement of Deficiencies D2099 ENDOCRINOLOGY CFR(s): 493.843(b) Failure to attain an overall testing event score of at least 80 percent is unsatisfactory performance. This STANDARD is not met as evidenced by: Based on record review and interview with laboratory personnel, the laboratory failed to achieve a score of 80% on one proficiency testing event . Findings include: Review of the American Proficiency Institute (API) records on 03/19/19 indicated that the laboratory got a 0% for thyroxine on the last testing event of 2017. Five of five samples had unacceptable results.
