Summary Statement of Deficiencies D0000 A recertification survey was conducted on May 10, 2022. Florida Pediatric Group clinical laboratory was not in compliance with 42 CFR 493, requirements for clinical laboratories. D6046 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(8) (b) The technical consultant is responsible for-- (b)(8) Evaluating the competency of all testing personnel and assuring that the staff maintain their competency to perform test procedures and report test results promptly, accurately and proficiently. This STANDARD is not met as evidenced by: Based on record review and interview, the Technical Consultant (Laboratory Director) failed to evaluate competency of testing personnel listed on the Laboratory's Personnel Report for two of two personnel for 2021, (A and B). Findings: Review of the Laboratory Personnel Report signed by the Laboratory Director on 05/10/2022 showed there were three employees listed on the form, the Laboratory Director and two Testing Personnel. The Laboratory Director was also listed as the Clinical Consultant and Technical Consultant. Review of Testing Personnel A's and B's competency evaluations dated 04/08/2021 showed they were signed by Testing Personnel A. On 05/10/2022 at 12:24 PM, Testing Personnel A stated it was her signature on the competency evaluations for 2021.
