Summary Statement of Deficiencies D0000 Forefront Dermatology SC DBA Advanced Dermatology & Skin Care Centre Forefront Dermatology Practice clinical laboratory is not in compliance with the 42 CFR part 493, Requirements for Laboratories. A CLIA recertification survey was conducted on 06/17/2021. D3011 FACILITIES CFR(s): 493.1101(d) Safety procedures must be established, accessible, and observed to ensure protection from physical, chemical, biochemical, and electrical hazards, and biohazardous materials. This STANDARD is not met as evidenced by: Based on direct observation and interview with the Laboratory Director and Histology Technician, the laboratory failed to dispose of Eosin and Xylene Substitute per the reagent container label instructions since beginning testing in 2019 (2019-2021). Findings included: During an interview on 6/17/2021 at 12:45 PM, the Laboratory Director and Histology Technician revealed that the Eosin and Xylene Substitute were disposed of down the drain of the sink. During tour of the lab, Eosin and Xylene Substitute reagents were located inside the flammable cabinet. The reagents had labels and instructions to "Dispose of contents/container to an approved waste disposal plant."
