This STANDARD is not met as evidenced by: Based on review of laboratory personnel records and interview with the histology supervisor, one of three testing personnel performing gross evaluations of histopathology specimens did not possess a current State of Nevada personnel certification. Findings include: 1. Review of laboratory personnel records revealed that one on three testing personnel performing gross evaluations of histopathology specimens did not possess a current State of Nevada personnel certification. The histotechnician began employment on 11/03/17 and left after March 2018. 2. The histology supervisor interviewed during the survey on 3/14/19 at approximately 11:30 AM confirmed the finding. The laboratory performs approximately 24,902 histopathology tests annually.
