Summary Statement of Deficiencies D6046 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(8) (b) The technical consultant is responsible for-- (b)(8) Evaluating the competency of all testing personnel and assuring that the staff maintain their competency to perform test procedures and report test results promptly, accurately and proficiently. This STANDARD is not met as evidenced by: Based on review of the personnel competency assessment records and interview with Technical Consultant (TC) #1, the TC #1 (Laboratory Director) failed to evaluate the competency of all testing personnel (TP) as required in 2017. Findings include: 1. On the day of survey, 10/18/2018, review of personnel 2017 competency assessment records revealed the laboratory failed to document 19 of 63 TP for their yearly competency assessment for laboratory testing performed in 2017: - 3 of 3 medical doctors reading Potassium hydroxide (KOH) and scabies preparation. - 6 of 6 CRNA's performing chemistry tests. - 10 of 10 respiratory technicians performing chemistry tests. 2. In 2017: 160 chemistry specimen were analyzed. 3. In 2017: 4 KOH and scabies slides were analyzed. 4. The TC confirmed the findings above on 10/17/2018 around 11:45 am.
