Summary Statement of Deficiencies D6054 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(9) The technical consultant is responsible for evaluating and documenting the performance of individuals responsible for moderate complexity testing at least annually, after the first year. This STANDARD is not met as evidenced by: Based on a review of the personnel records, policy and procedure manual, and an interview with the Technical Consultant (TC), the surveyor determined the TC failed to evaluate three of three Testing Personnel (TP) at least annually. The findings include: 1. A review of personnel records revealed there were no annual evaluations in 2017 for TP #1, TP #2, or TP #3. All had been previously qualified to perform high complexity patient testing since the previous survey conducted on 6/23/2016. 2. The policy entitled "Personnel Assessment" states "...Personnel will receive yearly evaluations after the first year of employment..." 3. In an interview conducted on 6/27 /2018 at 10:58 AM, the TC stated she could not find the competency evaluations for 2017. Thus the above noted findings were confirmed.
