Summary:

Summary Statement of Deficiencies D0000 This Statement of Deficiencies was created as a result of an on-site CLIA recertification survey conducted at your facility on August 5, 2026. The findings and conclusions of any investigation by the Division of Healthcare Purchasing and Compliance shall not be construed as prohibiting any criminal or civil investigations, actions or other claims for relief that may be available to any party under applicable federal, state, or local laws. D2006 TESTING OF PROFICIENCY TESTING SAMPLES CFR(s): 493.801(b) (b)The laboratory must examine or test, as applicable, the proficiency testing samples it receives from the proficiency testing program in the same manner as it tests patient specimens. This testing must be conducted in conformance with paragraph (b)(4) of this section. If the laboratory's patient specimen testing procedures would normally require reflex, distributive, or confirmatory testing at another laboratory, the laboratory should test the proficiency testing sample as it would a patient specimen up until the point it would refer a patient specimen to a second laboratory for any form of further testing. This STANDARD is not met as evidenced by: Based on a review of instrument printouts and laboratory records for American Proficiency Institute (API) Proficiency Testing (PT) 2025 Chemistry Core test events two and three, API PT 2026 Chemistry Core test event one, an interview with the laboratory supervisor on August 5, 2026, at approximately 12:00 PM, email correspondence received from the laboratory supervisor on August 18, 2026, at 12:12 PM PDT, and a telephone conversation on August 21, 2026, at approximately 9:45 AM PDT, the laboratory failed to ensure that proficiency testing specimens were tested in the same manner as patient samples for test results outside the assay range for High Sensitivity Troponin I (TNHI) for three of the six test events reviewed. Findings include: 1. The laboratory supervisor indicated that proficiency testing Statement of Deficiencies (X1) Provider/Supplier/CLIA Identification Number (X3) Date Survey Completed Name of Provider or Supplier Street Address, City, State -- 1 of 6 -- specimens are not diluted when results exceed the assay's upper range, and that patient specimens are diluted when High Sensitivity Troponin I results fall above the upper limit of the assay's analytical measurement range (AMR) (4.0 - 25,000 pg/mL). 2. The printouts revealed an error code of "assay range" indicating that the results were outside of the analytical measurement range of the instrument for TNHI. The laboratory failed to resolve the error code for the following proficiency test events: a. The laboratory reported TNHI results of 27,000 pg/mL for 2025 API Chemistry Core Event 2 sample CM-06. The instrument print outs indicated results of 27.298 ng/mL or 27,298 pg/mL which is above the AMR. b. The laboratory reported TNHI results of 69,816 pg/mL for 2025 API Chemistry Core Event 3 sample CM-12. The instrument print outs indicated results of 69.816 ng/mL or 69,816 pg/mL which is above the AMR. c. After receiving the PT evaluation, the laboratory re-ran sample CM-12 and obtained a result of 35.857 ng/mL (35,857 pg/mL) with the same ' assay range ' error code. The laboratory did not correct the error code or document any dilutions performed. d. The laboratory reported TNHI results of 34,283 pg/mL for 2026 API Chemistry Core Event 1 sample CM-01. The instrument print outs indicated results of 34.283 ng/mL or 34,283 pg/mL which is above the AMR. 3. There was no documentation of dilution and re-running of the PT samples with the Troponin I (CTNI) diluent in accordance with the manufacturer's instructions prior to reporting the results, as the laboratory indicated they did with patient samples. 4. The laboratory supervisor confirmed during an interview conducted on August 5, 2026, at approximately 11:45 AM that the High Sensitivity Troponin I PT results outside the high end of the analytical range were not manually diluted and repeated to ensure that the results were accurate and reliable. 5. The laboratory supervisor confirmed via email correspondence received on August 18, 2026, at 12:12 pm PDT, and during a telephone conversation on August 21, 2026, at approximately 9:45 am PDT that PT specimens are not diluted if they are outside the assay range high prior to reporting the results to API. The supervisor stated in both the email correspondence and during the telephone conversation that patient specimens are diluted when the High Sensitivity Troponin I results are outside the upper end of the assay range. According to the submitted CMS-116 form, the laboratory performs 5,835 chemistry tests annually.
