Summary Statement of Deficiencies D0000 An announced CLIA recertification survey was conducted at Gulf Coast Pain Consultants PLLC on 10/08/2024. The laboratory is not in compliance with 42 CFR Part 493, Requirements for Laboratories. The following is a description of the standard level deficiencies: D3011 FACILITIES CFR(s): 493.1101(d) Safety procedures must be established, accessible, and observed to ensure protection from physical, chemical, biochemical, and electrical hazards, and biohazardous materials. This STANDARD is not met as evidenced by: Based on observation and interview it was determined safety procedures to ensure protection from chemical hazards were not followed for four of four unopened bottles of Acetone. Findings include: During a tour of the lab on 10/08/24 at 10:55 AM, four unopened bottles of Acetone were observed on the open shelf under the laboratory hood. The safety label for the Acetone bottles indicated the contents were flammable and should be stored appropriately. The General Supervisor confirmed on 10/08/24 at 10:55 AM Acetone should be stored in the laboratory Safety Cabinet. The Technical Consultant confirmed via phone interview on 10/08/24 at 11:30 AM Acetone was to be stored in the laboratory Safety Cabinet.
