Summary Statement of Deficiencies D5429 MAINTENANCE AND FUNCTION CHECKS CFR(s): 493.1254(a)(1) For unmodified manufacturer's equipment, instruments, or test systems, the laboratory must perform and document maintenance as defined by the manufacturer and with at least the frequency specified by the manufacturer. This STANDARD is not met as evidenced by: Based on record review and staff interview, the facility failed to ensure weekly maintenance was performed on the microscope and centrifuge for 4 of 6 months in 2017. The findings include: The record review of the log titled "Microscopes, Centrifuges, Bench Cleaning, and Bottle Exchange Log" showed in April 2017, May 2017, and June 2017 there was no weekly maintenance performed on the microscope. Review of the same log showed in February 2017 and June 2017 the weekly inspection of rotor and buckets of the centrifuge was not performed. The interview with the General Supervisor on 6/26/19 at 11:17am confirmed the maintenance had not been performed.
