Summary Statement of Deficiencies D5209 PERSONNEL COMPETENCY ASSESSMENT POLICIES CFR(s): 493.1235 As specified in the personnel requirements in subpart M, the laboratory must establish and follow written policies and procedures to assess employee and, if applicable, consultant competency. This STANDARD is not met as evidenced by: Based on record review and interview, the laboratory failed to establish and follow a written policy for consultant competency for one of three technical consultants (SP8) reviewed. Findings include: 1. The "Point of Care Testing Program" (effective July, 2021) did not include a requirement for competency assessment for consultants. 2. Review of competency assessment for SP8 (technical consultant) indicated the last competency assessment was June 2018. 3. On 04/04/2022 at 12:50 pm, after requesting more updated competency assessments for SP8 (technical consultant), SP2 indicated that the most recent competency assessments for SP8 were from 2018.
