Summary Statement of Deficiencies D6046 TECHNICAL CONSULTANT RESPONSIBILITIES CFR(s): 493.1413(b)(8) (b) The technical consultant is responsible for-- (b)(8) Evaluating the competency of all testing personnel and assuring that the staff maintain their competency to perform test procedures and report test results promptly, accurately and proficiently. This STANDARD is not met as evidenced by: Based on an interview with the testing personnel and a personnel record review, the laboratory director, who is the technical consultant, failed to evaluate the competency of the testing personnel performing complete blood counts (CBCs), urine microscopic, and wet mount tests since the last survey on June 6, 2016. Findings: 1. A review of personnel records revealed one (1) out of 1 testing personnel did not have competency assessments completed for the CBCs, urine microscopic, and wet mounts since the last survey on June 6, 2016. 2. An interview conducted on January 9, 2018, at 12:15 P. M., with the testing personnel, confirmed that competency assessments were not performed.
