Summary Statement of Deficiencies D3031 RETENTION REQUIREMENTS CFR(s): 493.1105(a)(3) Analytic systems records. Retain quality control and patient test records (including instrument printouts, if applicable) and records documenting all analytic systems activities specified in 493.1252 through 493.1289 for at least 2 years. This STANDARD is not met as evidenced by: Based on record review and staff interview on 08/18/2021, it was determined the laboratory failed to retain patient result records for two (2) years. Findings include: 1.) Review of patient test records revealed that urine microscopic test results were not archived or available for review. 2.) Interview with technical consultant at 11:45 AM on 08/18/202 revealed the laboratory failed to have a system in place to ensure patient result records are kept for two (2) years.
