This STANDARD is not met as evidenced by: Based on lack of documentation for competency assessments for the years 2022, 2023, and 2024 and interviews with the technical consultant and testing personnel; it was determined that the technical consultant failed to perform and document the performance evaluation for one of the testing personnel and assuring that they maintained their competency to perform test procedures promptly, accurately, and proficiently.
