This STANDARD is not met as evidenced by: Based on a record review and an interview with the technical consultant (TC) and the quality manager, the laboratory failed to establish and follow a policy to assess two out of two technical consultant competencies since the last survey on 06/11/2021. Findings include: 1. A review of the laboratory's competency records revealed that the laboratory failed to establish and document two out of two TC competencies. 2. An interview with the TC and the quality manager on 01/04/2024, at 1:30 p.m., confirmed that the TC competencies were not established or performed.
