This STANDARD is not met as evidenced by: Based on record review and interview with the Office Manager, the laboratory failed to have competency evaluations for Personnel #B for 2 out of 2 years (2017-2018) reviewed. Findings Included: Review of employee competency evaluations found no competencies for Personnel #B ( who is the Clinical Consultant, Technical Supervisor, General Supervisor, and Testing Person for the laboratory) for 2017 and 2018. Interview on 03/28/19 at 4:00 PM with the Office Manager confirmed that there were no competency evaluations for Personnel #B.
