This STANDARD is not met as evidenced by: Based on review of American Proficiency Institute (API) proficiency testing (PT) records and interview with Testing Personnel # A (TPA), the laboratory failed to submit PT results before the due date for 1 out of 6 events for Routine Chemistry reviewed (2017-2018). Findings include: Review of API PT records revealed a score of 0% for pH (PH) Blood gas , Partial pressure Oxygen (PO2) and Partial pressure Carbon dioxide (PCO2) and an overall score of 0 % for Routine Chemistry for the second event of 2017. During an interview on 2/19/2019 at 9:30 AM, the TPA confirmed that the laboratory failed to submit the results within the time frame for the event of reference.
