This STANDARD is not met as evidenced by: Based on record review and interview, the laboratory failed to dispose of chemicals per manufacturer's instructions for 2 out of 2 years, (1/8/17 to 1/8/19) reviewed. Findings: Review of the manufacturer's instructions on the bottles of Bluing Reagent, Clear-Rite 3, Eosin-Y, Flex 100, Hematoxylin 7211, and 70% Dehydrant all stated, "Dispose of contents/container to an approved waste disposal plant." During an interview on 1/8/19 at 1:22 PM, Mohs Technician confirmed that the chemical reagents are disposed in the sink drain.
