This STANDARD is not met as evidenced by: Based on review of testing personnel records and interview with the Laboratory Director, the Technical Consultant failed to assess the annual technical competencies for 4 out of 4 testing personnel (TP) in 2017 and 2018. The findings include: 1. Review of the testing personnel records for 4 out of 4 TP revealed the Technical Consultant failed to assess the annual technical competencies for 2017 and 2018. 2. Interview with the laboratory director on November 14, 2018 @ 12:30 pm confirmed the Technical consultant failed to assess the annual technical competencies for 2017 and 2018.
