This STANDARD is not met as evidenced by: Based on a direct observation and an interview with the laboratory lead on 5/26/2026, the laboratory failed to discontinue the use of expired Mohs reagents since April 2026. The findings include: 1. A direct observation of the laboratory's reagents on 5/26/2026 identified that the laboratory failed to discontinue the use of the Eosin-Y Vintage stain lot 208517 expiration 2026-03-31 prior to the expiration. 2. An interview with the laboratory lead on 5/26/2026 at 11:12 am confirmed the above finding. 3. The laboratory reports performing 275 Mohs slide examinations annually.
