This STANDARD is not met as evidenced by: Based on surveyor review of competency assessment records and interview with the technical consultant, the technical consultant did not document site specific direct observation of routine patient testing for nine of ten clinic testing personnel in 2022 and 2023. Findings include: 1. Review of competency assessment records showed no evidence of direct observation performed at this clinic site for testing personnel. 2. Interview with the technical consultant on June 4, 2024, at 10:30 AM confirmed the technical consultant did not document direct observation at this clinic site for routine testing of testing personnel.
