Summary Statement of Deficiencies D5221 EVALUATION OF PROFICIENCY TESTING PERFORMANCE CFR(s): 493.1236(d) All proficiency testing evaluation and verification activities must be documented. This STANDARD is not met as evidenced by: Based on the surveyor review of the Biannual Assessment (BA) records and interview with the Testing Personnel (TP), the laboratory failed to evaluate Prostate Specific Antigen (PSA) and Urine Colony count tests twice annually in the calendar years 2017 and 2018. The findings include: 1. There was no evidence of documented review of BA samples in 2017 and 2108. 2. This deficiency was cited on survey preformed on 2/28/17. 3.
