Summary Statement of Deficiencies D6125 TECHNICAL SUPERVISOR RESPONSIBILITIES CFR(s): 493.1451(b)(8)(v) The procedures for evaluation of the competency of the staff must include, but are not limited to assessment of test performance through testing previously analyzed specimens, internal blind testing samples or external proficiency testing samples. This STANDARD is not met as evidenced by: Based on record review and staff interview, the Technical Supervisor failed to evaluate competency of testing personnel performing mycology testing with an assessment of test performance in 2017 and 2018. Findings include: 1) Review on 4/17 /2019 of personnel records revealed no documentation of an assessment of test performance in 2017 or 2018 for one of one testing personnel (Staff A) performing microscopic potassium hydroxide (KOH) slide preparation examinations. 2) Interview on 4/17/2019 at 2:00 p.m. with Staff B (Corporate Office Personnel) revealed Staff A's first day was 10/21/2016. 3) Interview on 4/17/2019 at 2:00 p.m. with Staff C (Histotech, Testing Personnel) confirmed annual competency assessments of test performance had not been completed for Staff A.
